Welcome to the Tiny Smiles—A Give Kids A Smile Program Post-Program Survey. Please complete this survey after reviewing the program materials to share your thoughts on them. Your feedback will be kept confidential. We appreciate your input, as it plays a key part in improving our health educational programs.

WHO CAN ENTER: NO PURCHASE NECESSARY TO ENTER OR WIN. The Tiny Smiles Post-Program Survey Sweepstakes (“Sweepstakes”) is open only to (i) dental and medical professionals who provide services to children ages 0 to 5 and/or to their parents or caregivers and (ii) educators who work with children ages 0 to 5 and teach in a public school, an accredited private school, or a home school in compliance with the laws of the teachers’ primary state of residence (including daycares and preschools), and who are over 18 at the time of entry, who are residents of the 50 United States (or the District of Columbia), and who receive the print program materials in the mail, view the program materials online, or receive the email invitation to participate in the Tiny Smiles Post-Program Survey (“Survey”) and review the materials, and submit a completed Survey. Void where prohibited.

HOW TO ENTER: Entrants must submit a completed Survey, which is hosted online at scholastic.com/givekidsasmile/survey, between 12:01 a.m. Eastern Time (“ET”) on October 1, 2019, and 11:59 p.m. ET on April 3, 2020 (the “Entry Period”).

PRIZES: All eligible entrants who complete a Survey and submit their information will be entered into a random drawing for a chance to win. Fifty (50) winners will receive a library of books from Scholastic valued at $100. For more information, see Official Rules.
